I have been trying to configure a form to be used for Decent Homes assessments within the Housing Assistance module of CX. I've tried a lot of different ways, but I can't get the forms to work in our assistance cases. When creating an ad Hoc form, it allows me to choose the Entity Type and Form Definition, but there is then no reference to choose in order to generate the form. On the config the main entity is Housing Assistance Case.

Has anyone had much success with HA Forms?
